How do you know if you’re seriously hooked on nicotine? And when should you think about meds?

I’ve seen a few people asking how to tell if their smoking habit is actually a heavy dependence, and whether it’s time to bring in something stronger than willpower. So here’s my take – based on what I’ve learned from the Quitting Smoking clinic and some real-world experience.
There’s this standard questionnaire called the FTND (Fagerström Test for Nicotine Dependence). Six questions, gives you a score. But honestly, most long-time smokers don’t even need the full test. Just ask yourself two things:
- How soon after waking up do you need your first cigarette?
- How many do you smoke per day?
If you’re lighting up within 5 minutes of opening your eyes, or you’re smoking more than a pack a day (20+), chances are you’re in the moderate-to-severe range.
Why those two questions? Because overnight, your nicotine level bottoms out. The urgency you feel in the morning is a dead giveaway of how much your brain is craving it. According to China’s clinical smoking cessation guidelines, heavy dependent smokers who try to quit with sheer willpower have a success rate below 5%. But with medication, that rate goes up by 1.5 to 2 times.
So, do you need medication? Here’s my rule of thumb:
- FTND score of 7 or higher
- Smoke 10+ a day
- Or you’ve tried quitting cold turkey a few times and failed
Then don’t torture yourself. Go see a doctor at a smoking cessation clinic. Meds like nicotine patches, gum, or varenicline (Champix) are well-proven and widely available. It’s not a shame thing – it’s just using the right tool for the job.
